Why purchase personal accident protection? 

The VBA accident protection can be used by itself to cover accidents or can coincide with your existing health insurance policy. As consumers, we often purchase PPO policies with high deductibles or high annual maximum out-of-pocket amounts in order to keep down our monthly premiums. The VBA accident protection can help to minimize those out of pocket costs from your regular health insurance plan, in the event of an accident.

Offset your high deductible health plan  (Perfect for those with HSA Plans)

Do you have an HSA or simply a high deductible health plan?  The VBA accident benefit plan fills the gap on your high deductible health insurance plan. For example, many HSA plans have a $2500 deductible or higher.  If a family member has an accident, the $2500 benefit from VBA would offset your health plan deductible. If you have teenagers, there's no doubt you have made a trip or two to the doctor's office or emergency room for the occasional skateboard, snowboard, snow ski, surfing, mountain bike or sports injury.    24 Hour Accident Protection
 
  • $2,500,  or   $5,000  or  $10,000 of Accident Coverage for Each Family Member per Occurrence For Medical Services.  Your choice of benefit level ($2500 or $5000  or $10,000 per accident).

Medical Services means the costs for: Medically necessary treatment by a physician, nurse, dentist, hospital room and board, outpatient surgery, use of an ambulance, dental work to sound natural teeth, drugs, medicines, diagnostic tests and x-rays, oxygen, casts, splints, crutches, blood plasma, treatment performed by licensed medical professional and rental of durable medical equipment.

  • Use Any Doctor, Emergency Room or Hospital

  • Pays Directly to You, unless you assign

  • $250 deductible for $10,000 plan (Premier Plan)  or
    $100 Deductible for the $5,000 or $2,500 plans (Basic Plus and Basic Plans)

 

   Accidental Death and Dismemberment

 

  • $5,000 Coverage for Each Family Member

If a covered family member's injury in a loss, as defined in your certificate of coverage, within one year after the accident causing the loss, the carrier will pay per the schedule of benefits for loss of life.  Also benefits for loss of limb and sight are shown in the schedule of benefits.

 

   Emergency Air Ambulance

 

What is it? As members of Lifeguard Emergency Rescue, this plan provides greater peace-of-mind to individuals and groups engaged in a wide variety of activities. It gives all eligible persons the security they need and deserve.

If, as a result of injury, a member incurs covered expense starting within 90 days from the date of the accident causing the injury, we will pay, with no deductible and not to exceed the overall maximum benefit amount of $4,000.00, for Emergency Air Ambulance Transportation Only.

  • Coverage is worldwide

  • Transportation by air only

   How do I enroll?
 

Step 1 - Download enrollment form (two choices)

 

Step 2 - Print enrollment form

  • If you don't have the Adobe Acrobat Reader, you can download it here.
  • Or call us (800-610-6418) or email us and ask us to fax or mail it to you.

 

Step 3 - Complete the enrollment form

  • Complete the simple one-page enrollment form, sign and fax it to:  (949) 203-8786
 

Step 4 - Decide on a payment option

  • You can pay via automatic monthly checking bank draft  or by monthly credit card debit.

 

Step 5 - Fax enrollment form to (949) 203-8786

  • Simply fax your signed enrollment form to us at Fax Number:  (949) 203-8786.
    Include a voided check if paying by monthly checking account deduction.

 

Note:  Be sure to sign the application in two places.  The signature states that all information is correct and the second signature authorizes the automatic payment by either automatic bank draft or monthly credit card payment.

Don't forget to include a voided check if using the automatic monthly bank draft option!
