simplified plan that is affordable too. It also includes dental!
If you ride a skateboard or snow board, or are
just young or are young at heart, this plan is for you. Not only does the plan
offer a low copay for doctor visits, and includes dental, but it also
allows you emergency room visits for a flat $100 copay.
Doctor visits are available for a small copay,
depending on the plan selected (deductible is waived).
Also, once you meet your deductible, it gives you 100%
coverage for the remaining calendar year.

Blue Cross
RightPlan 40 PPO
This is a no-deductible PPO!
Plan designed for healthy individuals (this
is not a family plan)
Blue Cross of California RightPlan 40 is an individual
plan that does not have a deductible. That means Blue Cross immediately
starts paying for inpatient, outpatient and professional services at 60%,
you pay 40%. Doctor visits are covered for a flat $40 copay. This
plan has three choices for prescription drug coverage, namely - no drugs,
generic only, and comprehensive prescription drug coverage. Monthly
premium varies depending on selected drug coverage.

Comprehensive PPO Plans

Cross of California PPO Plans)
Blue Cross' comprehensive PPO plans are called
the PPO Share plans. The two PPO Share plans that we recommend are
the PPO Share 5000 and PPO Share 2500 plans.
These plans allow you to adjust your deductible to make
your monhtly premium affordable.
- Pay $25 or $75 copay for basic HealthyCheck health
screenings.
- Routine mammogram, PSA and Pap tests are 20% of the negotiated
fee (deductible waived).
- Well-Child (up to age 6) at 50% of negotiated fee
(deductible waived).

Basic PPO 1000 and Basic PPO
2500 The Blue Cross Basic PPO 1000
and the Blue Cross Basic PPO 2500 plans are catastrophic plans that are designed
to guard against major hospital and
doctor bills, yet require you to pay for routine office visits and professional services
out of pocket, but at a discounted rate. The hospital deductible is
either $1000 or $2500, after which they pick up 80% until you reach the out of pocket
maximum of $3500, at which time they pay 100% for the rest of calendar
year.

HMO Plans from Blue Cross of
California ...
Health Maintenance Organizations
If you are looking for comprehensive health care
coverage, a Blue Cross HMO may be right for you. In a Blue Cross
HMO, you select a doctor to serve as your Primary Care Physician. Your
Primary Care Physician coordinates all of your health care, which means
that he or she treats you directly, and when necessary, manages your
referral to specialists. Most importantly, the HMO will give you the
least amount of "out-of-pocket" costs when you need to go to a health care
provider.

Select HMO - Best
selling HMO in CA.
The Blue Cross Select HMO is one of the best selling
HMOs because it is one of the lowest priced HMO's available in California.
The provider network for this plan is smaller than their normal HMO
network, so you may want to
check for doctors and hospitals in your area before deciding on this
plan (see "Select HMO" network).
The BC Select HMO has no annual deductible,
unlimited lifetime benefits and $25 office visit copays. It also covers
both generic and brand name drugs ($250 annual brand name deductible).

Individual HMO
The Individual HMO has no annual deductible,
unlimited lifetime benefits and $10 office visit copays. If you are
looking for Blue Cross' most comprehensive health care coverage, this plan
may be the right choice for you.

Blue Cross Saver HMO
The Saver HMO
is similar to the Individual HMO above, however it has a $1,500 annual deductible that applies to hospital and surgical
services. This deductible is waived for emergency admittance. The HMO
Saver Plan also has unlimited lifetime benefits and $10 Office Visits.
As the name indicates, the monthly premium for this
HMO is less than the Individual HMO.
