We recommend the
Anthem Blue Cross Dental Blue 100 Plus Plan
The Dental Blue 100
Plus plan is a PPO dental plan that covers preventative services at 100%
(exam/cleaning/x-rays). There is a $50annual deductible on this
plan, however it does not apply to preventative services.

Annual deductible: $50
Annual Maximum Coverage: $1000
Preventative Services: No charge (deductible waived)
Minor dental services: You pay 20% after the $50
deductible
Orthodontics: Not covered.
For a single applicant, the premium is
approximately $38 to $45 per month.
Couple is approximately $74 to $79 per
month.
Family plan is approximately $127 per
month.
RATES WILL VARY, DEPENDING ON THE COUNTY
YOU LIVE IN.
How the plan works
This is a PPO dental plan. You can go any
dentist in the
Blue Cross dental Blue PPO Provider Network, or you can go to
dentists outside the network, however services provided by non-network
dentists do not receive the Blue Cross pre-negotiated discount.
Therefore you will have more out of pocket costs when going to
non-network dentists.
If you are ready to apply, simply
download the application, print it, follow the directions and mail
the application and first month premium back to the address listed in
the directions that come with the application. If you have any
questions, please call (949) 394-7676.
For faster service you may fax the
application back to FAX 949-203-8786, with a voided check and
completed automatic bank draft authorization form. Using this
method you first month's premium (and successive payments) will be
deducted from your checking account automatically on a monthly basis.
